Required Forms
If a substitution needs to be made, please complete the Substitution form (pdf link) and turn it in upon check-in. If no proof of payment is found for the original registrant, the substitute delegate will be required to register on-site and pay on-site fees.
Potential delegates under 18 years of age are required to download the liability form (pdf link) before the conference and have their parents/guardians sign off on the necessary fields before turning it in during check-in. Failure to do so will result in forfeiting associated fees and participation in the conference.
