Online Registration IS CLOSED!!!
(If you do not pre-register online, you will have to pay the on-site registration fee. For those who successfully pre-registered, money sent must be postmarked and received to avoid on-site charges)


The Pre-registration member fee of $20 is due within 2 WEEKS of your registration date. The non-member fee for Pre-registration is $25. If payment is not received within the two weeks** of submitting this form, you must pay either the on-site fee of $30 or $35 respectively. A reminder notification will be sent if payment is not received within this time. (**"Last Call" Registrants must have money received by March 7th, 2008)

Please send payment in the form of an institutional check or money order, as no personal checks will be accepted. Make money orders/checks payable to 'Filipino Intercollegiate Networking Dialogue, Inc.' with memo: Registration Money, and send to:

March 3, 2008 - ALL Pre-Registration Ends
March 7, 2008 - ALL Pre-Registration Payment Due**
March 14, 2008 - On-Site Registration/Check-In open from 6:30 pm to 9:30 pm
March 15, 2008 - On-Site Registration/Check-In open from 8:00 am to 12:00 pm

DEADLINE
All Pre-registration payments need to be postmarked by March 7, 2008. All payments received afterwards will be returned and the delegates/delegations will have to register on-site and pay on-site registration fees.



GROUP PAYMENT
If one lump sum payment of registration dues is to be made for several individuals, plese remember to include the names of all the covered registrants along with your check/money order.



REFUNDS
No requests for refunds will be accepted. If an individual who has paid is unable to attend, we encourage that a substitute be found.



SUBSTITUTION
If a substitution needs to be made, please complete the Substitution form (.pdf) prior to check-in and turn it in upon check-in. If no proof of payment is found for the original registrant, the substitute delegate will be required to register on-site and pay on-site fees.



ON-SITE REGISTRATION

FIND, Inc. Member Schools: $30 /delegate
Non-FIND, Inc. Member Schools: $35 /delegate


On-site registration will be located at Adele Stamp Student Union Building Grand Ballroom. Accepted forms of payment include institutional checks, bank checks, and cash. Personal checks and money orders will not be accepted.

MINORS
Potential delegates under 18 years of age are required to download the Liability Form (.pdf) prior to check-in and have their parents/guardians sign off on the necessary fields before turning it in during check-in. Failure to do so will result in forfeiting associated fees and participation in the dialogue.



CHECK-IN

All delegates are required to check-in at the Adele Stamp Student Union Building during the times specified above. Please bring two forms of photo-ID to be shown upon check-in. Delegates from Member Schools MUST provide their school ID of their member school to check in.

Delegates will be asked to sign the liability form; failure to do so will result in forfeiting associated fees and participation in the dialogue. If you wish to review the form in advance, it is available for download in PDF format (.pdf). The liability form is not to be signed beforehand (minors excluded).

Upon successful check-in, delegates will receive their badge and delegate packet.

The registration chairs will be on hand to resolve any issues that may arise.
